Also if my student needs medicine during the day once a week, how does that work? is there a nurse's office that can accomodate?
To ensure your student receives medication during the school day at Claire Lilienthal Elementary, specific procedures must be followed.
Documentation Requirements Both you and your child's healthcare provider must complete the SFUSD Medication Form, which is required for all medications, including prescription drugs and over-the-counter medications. This form must be renewed annually.
Medication Delivery and Administration You or a designated adult must personally deliver the medication to the school office in its original pharmacy-labeled container. A school nurse or a trained staff member will administer the medication. If your child is authorized to self-administer, this must be noted on the Medication Form by both the parent and the doctor.
Nurse Availability The school provides access to a "Nurse of the Day" and on-site nurses who can assist with medication administration. For specific inquiries regarding your child's needs, you can contact the school office at (415) 749-3516 during office hours, which are Monday to Friday from 9:30 AM to 4:00 PM, with earlier release on Wednesdays.
For more detailed information, you can refer to the SFUSD Student and Family Handbook.
