Huntington Seacliff Elementary is committed to ensuring that students with documented health conditions receive the necessary academic accommodations. These accommodations are facilitated through Section 504 plans and Individualized Education Programs (IEPs), which are designed to provide equal access to educational activities for students with physical or mental impairments.
Application Process
To set up accommodations, follow these steps:
- Submit a Written Referral: A parent or guardian must send a formal written request for evaluation to the school principal or psychologist, indicating it is for a 504 plan or special education services.
- Provide Medical Documentation: Include documentation from a licensed healthcare professional detailing the diagnosis and its impact on major life activities.
- Student Study Team (SST) Meeting: The school will convene a meeting with parents and specialists to discuss the student's needs and determine if further assessments are necessary.
- Assessment and Evaluation: If an assessment is recommended, the district will provide an assessment plan within 15 days, and typically complete the evaluation within 60 calendar days.
- Plan Development: If eligible, a team will create a formal 504 plan or IEP outlining specific accommodations.
