- Written Authorization: Both a parent/guardian and a licensed physician must provide written orders for the medication.
- Required Form: The Parent/Guardian and Authorized Health Care Provider Request for Medication form must be completed.
- Original Packaging: Medication must be delivered in its original, pharmacy-labeled container.
- Scope: This applies to both prescription and over-the-counter medications.
